  1. At Centennial Express, I developed a SQL database with a MS Access front end.   The company managed the
    movement of containers on the rail portion of an international shipment.  They rented empty containers and
    constantly had to track their movements on several railroads.

  1. In work at West Suburban Physician Services I prepared many reports using data from the main database
    (a Sybase SQL database application) and files provided by external sources.  Here are screen shots of that allowed
    administrators to adjust the fees for various CPT procedures based on Medicare rates and the quantity done.
    The blank box on the Fee Comparison was where the new "price" was entered and then typed into a file.
     I later did an insert on the database to put in the new fee schedule in place.  (Because of HIPAA I did not
     retain any of the database info that included patient information.  I did many of these as we scrubbed the database
    of info that did not agree with HMO monthly data reports.)

  1. HTML based report

As a teacher I developed a method to allow parents and students to view progress in their course.

The data is old but shows the idea.  Your Grade Use the sample.
